Healthy Families Health Insurance
The 4C Council helps income-eligible families enroll into the "Healthy Families" program, a low-premium-based health insurance program for low income families with children up to 21 years of age. This is accomplished by collaborating with a number of organizations, including

• Santa Clara Family Health Plan
  
• Valley Medical Center
  
• Blue Cross
  
• Kaiser Permanente

Additionally, the 4C Council assists in educating families about their insurance options and in the completion of the application and forms.
